Wellicht ten overvloede, maar alle,
de door
ons
vertegenwoordigde,
kuuroorden staan onder controle van de ministeries
van
gezondheid in het bewuste land. Vanzelfsprekend staan de geneeskundige afdelingen
onder leiding
van gediplomeerde artsen. Als
u nimmer een balneo kuur of dergelijke
verzorging hebt gevolgd, kunnen wij ons voorstellen dat u er ambivalent tegenover
staat. Toch is
hier geen reden voor. Ook verscheidende Nederlandse academisch
medische centra,
als
het
Leids
Universitair
Medisch
Centrum en Academisch Ziekenhuis
Maastricht, hebben
onderzoek gedaan naar de effecten. Deze onderzoek(st)ers zijn
eveneens tot de conclusie
gekomen dat medische kuurreizen daadwerkelijk helpen.
Graag willen wij u
"inwijden". Bij aankomst wordt een medische
check-up uitgevoerd
door een arts. Afhankelijk van uw ongerief en gekozen kuuroord, kan een
dermatoloog,
orthopedist, reumatoloog, internist of andere specialist u
onderzoeken. De dokter
bepaalt aan de hand van dit onderzoek, rekening houdend met uw conditie en in
samen
met u het behandelplan. De gekozen kuurbehandelingen vinden gedurende de
hele week plaats, behalve op zon- en feestdagen. Tussen de procedures kunnen
tijdsintervallen zijn gepland om de werking
van de kuurbehandeling te
optimaliseren. U rent dus niet van de één naar de andere kuur, maar u hebt
vaak ruim
de tijd
om
te
genieten
van
de
andere
voorzieningen,
als
zwembad,
terras,
sportfaciliteiten
of
een
goed
boek.
U
beslist.
De behandelingen sorteren na enige tijd effect. Als u de geadviseerde tijdsduur
(zie bij het kuuroord in kwestie) doorloopt zal het resultaat maanden na beëindiging
van de kuur nog goed merkbaar zijn.
Tijdens uw kuurarrangement in uw vrije tijd zoals op zondagen en bij voldoende belangstelling
van de kuurgasten worden vaak excursies
verzorgd. Gedurende de week kunt u ook aan andere activiteiten deelnemen als dansavonden, concerten, enz.
De vakantiemaanden, juli en augustus, zijn voor veel kuuroorden aanleiding om muziekfestivals te
organiseren. U ziet uw algehele welzijnsgevoel staat voorop. Vandaar onze slogan:
op weg naar weldaad!
Speciaal zijn ook
drinkkuren, zoals
onder meer in Bardejov, Karlsbad en Mariënbad worden gebruikt.
Het geschikte bronwater wordt voornamelijk voor spijsverteringsorganen als
maag, darmen, lever, gal, nieren, milt en urinewegen gebruikt. Daarnaast zij
sommige bronnen bijzonder nuttig bij trombose, problemen met de ademwegen of
schildklier. Andere thermale waterbronnen zijn goed voor uw hart en bloeddruk
of bij anemie. Ook zijn er wellen die kunnen helpen bij genealogische
kwesties, overgang en aanverwante hormonale perikelen. Om hiervan optimaal
te profiteren is het belangrijk dat u het bronwater op gezette tijden drinkt.
De arts zal u voorschrijven welke bron goed voor u is alsook hoeveel en het tijdschema.
Men dient met kleine nipjes te drinken. Het beste is om tijdens het drinken
ontspannen te lopen, zodat een beker (2 dl) tussen de 3 en 10 minuten leeg is. |
Over de
auteur: dr. Jaroslav
Mikula is specialist in psychiatrie, balneotherapieën en herstelwijze. Zijn carrière
begon in Tsjechië. Thans is hij medisch directeur van kuureiland Piestany.
Met meer dan 120 publicaties op zijn
naam is hij een expert bij uitstek. Zijn literatuurlijst behelst onderzoeken naar:
Hippotherapie,
applicatie van bronwater, herstelprogramma's voor heup- en kniegewrichten, methodes voor de
ziekte van Parkinson, gewrichts- en evenwichtsstoornissen. Ook heeft hij studies
gedaan naar de
ruggengraat, herstelprogramma's na rugoperaties en
cardiovasculaire aandoeningen. Dr. Mikula

PIESTANY
SULPHUR MUD - FULL BODY OR PARTIAL MUD PACKS
Sulphur
mud is a unique peloid both in the European as well as worldwide scale. Its
base is a homogenized sediment precipitating in the river Vah by-pass branch on
the site of the current hot springs of the sulphuric thermal mineral water.
Saturation of the mineral water without air creates a unique environment
inhabited by desulphurisation bacteria (Desulfovibrio desulfuricens) and by
other b acteria with metabolism dependent on the presence of
sulphur
(Thiobacterium, Thiospirillum). The mud has important antifugal features. It is
prepared for balneological application using the original technology, which
cause it to mature and regenerate, which is accompanied by multitude of chemical
and biological reactions. The mature mud has a butter-like consistency, it is
greasy, malleable, with good heat conducting features, it cools down four times
faster than water, compared to which the former has 350x higher viscosity. The
colour of the mature mud varies from steel blue to black. Mud application
temperature is approximately 45°C (from 44° to 46°C); duration of partial or
total wrap is 20 minutes. The thickness of the mud wrap layer applied on the
body is approximately 5 cm (3 - 8 cm); the mud consumption per the total wrap is
up to 60 kg. The wrap is not applied on the head, frontal parts of the throat
and chest. The patient is under the permanent medical supervision. The wrap
causes a temporary local, as well as total body overheating, which depends on
the application area of the wrap. The mud pack causes the reduction of local
swellings around joints, reduction of the muscle tension, better nutrition of
cartilage, connective tissues and intervertebral discs. It strengthens
anti-inflammatory activity and immune processes within the body, preventing
degradation of elastine, collagen and hyaluronane, and thus improving the
function of cartilage, connective tissues and joint motorics. The procedure ends
with a shower and up to 15-min dry wrap. The procedure and scale of mud
application are subject to the doctor's prescription.
TUB
BATHS
Generally exerts mechanical effects (hydrostatic pressure and buoyancy, which
both act as an elastic bandage, eliminating effects of gravity through lifting
the body, thus supporting the vascular and lymphatic circulation) and thermal
effects (depending on the temperature, duration, and the surface area and mass
of the body), which through affecting the thermoregulatory processes mobilizes
the nervous, endocrineous, cardio-vascular, respiratory and immune system. The
specific chemical effect of the bath depends on the composition of the used
water. Piestany mineral water is natural, weakly mineralized
sulphate-hydrocarbonate, calcium-sodium, and hypo tonic sulphuric thermal water.
Save the head, during baths in tubs the whole body or its part are submerged in
the water.
a) Bath in thermal mineral water is used
mainly for the dermatic penetration of clupeids from mineral water, which
results in deposits of sulphur in the skin. From there the sulphur gradually
migrates to other structures, mainly as chondroitinsulphate to the connective
tissues (ligaments, cartilage, sinews, etc.), and it is further absorbed in the
body as cysteine, methionine or gluthation. The sulphur inhibits the degradation
of elastine, collagen and hyaluronane, thus reducing he blood pressure, and
causing the dilatation of blood vessels; it positively affects the volume of
Vitamin C in the plasma, and supports the destruction of free oxygen radicals.
The bath has the total relaxation and tranquillizing effect. The temperature of
the mineral sulphuric bath is usually 36°38°C (in exceptional cases
up to 40°C), it lasts 20 minutes (sometime a doctor may prescribe only 15
minutes for health reasons), and it ends with a dry wrap up to 15 minutes. The
procedure is subject to the doctor's prescription, but it could be prescribed
also to persons with severe mobility handicaps, and also to patients who cannot
be prescribed the mirror pool (since for them it is counter-indicated). The bath
with temperature 37°C and less is listed as freely purchasable procedure.
b) Bubble bath is prepared through
saturation of the mineral water with the temperature 36°- 37°C using
the compressed air (0.15 MPa), which is fed to the perforated matt placed on the
bottom of the tub. Air bubbles rise up from the bottom to the surface, and cause
soft massage of the skin. The bath lasts 20 minutes, and it ends with a dry wrap
up to 15 minutes. The bath has a sedative effect, it facilitates the patient's tranquillization,
and it is suitable in case of insomnia and neurotic problems. It is either
prescribed by the doctor, or freely saleable.
c) Tub carbon bath is prepared through
saturation of the mineral water cooled down to 32°- 34°C (hypothermic
bath) with a carbon dioxide used in food industry, taken from the pressure
vessel (2 - 3 atm), and applied through a saturator. The carbonic bath has to
contain at least 1000 mg of free CO2 per 1 kg of water. In order to dissolve in water a sufficient volume of
CO2, the temperature of water has to be lower, the tube has to be filled from
the bottom up, and the position of a patient in the bath should be fully
relaxed, with head over the edge of the tub. The water must not whirl since that
would cause an excessive release of dissolved CO2, and its cumulating just over
the surface of the water (the risk of toxic affect of an excessive concentration
of CO2, which is heavier than air). The surface of the bath is thus covered,
especially on the place below the patient's face. During the bath CO2 bubbles
are generated on the skin (creating sort of gas film), and the gas is strongly
diffused and absorbed (depending on the quality of the sk61 and its blood
supply). The carbonic bath reduces the blood pressure, improves the efficiency
of the heart action, and significantly improves the congestion in the body with
the simultaneous elimination of its thermal overload. The initial feeling of
cold is in some 10 seconds replaced by the feeling of pleasant warmth, which is
the result of active hyperemia, boosted activity of thermo receptors and contrariwise
reduced activity of cold receptors. The bath is suitable for coronary-vascular
and nerve diseases. It lasts from 15 to 20 minutes, followed by the dry wrap up
to 15 minutes. It is either prescribed by the doctor, or freely saleable.
SAUNA
Is a bath in hot and dry air (temperature 60°- 90°C, but low moisture 10 -
30%). Air temperature in sauna is typically divided into bands according to the
height of
individual benches (lowest next to the floor, approximately 40°C, highest next
to the ceiling, up to 110°C). In the height 150 cm and 100 cm from the heat
sources it should reach 80°C, with the relative moisture not more than 15%. In
such environment the sweat evaporates well, resulting in an effective cooling of
the organism. A constituent part of the course of sauna is the subsequent
chilling with cold water (washing, showering, immersion bath, swimming). This
cycle is repeated 2 - 3 times and ends with relaxation or walking. In addition
to that the skin in sauna is exposed to mechanical irritation (flogging with a
leafy birch branches), which results in a histamine reaction, and dilatation of
capillaries. Sauna treatment includes also the so-called shock-steam waves
(sudden increase of air moisture, e. g. by pouring water over the hot stones),
which changes the evaporation of sweat and the further increases the temperature
of the organism core. Sauna supports the body conditioning, and it is a suitable
prevention against cold-caused diseases, positively affecting the functional
circulatory malfunctions, light rheumatic diseases, and some allergic diseases.
Recommended periods: 20 min stay in sauna, 5 min cooling + flogging, 15 min stay
in sauna (+ steam shock), 5 min cooling + flogging, 10 min stay in sauna (+steam
shock), 5 min cooling + flogging (60 minutes), final rest after sauna 60 minutes.
CLASSICAL
MASSAGE
Manual
application of targeted mechanical stimuli, the so-called touches, which contain
not just kneading action (friction, wiping, grinding and kneading), but also
striking actions (pounding and vibration). The massage supports evacuation of
surface veins, lymphatic vessels, secretion of sweat glands, normalization of
skin tonus, better blood flow in the region subjected to massage, absorption of
swellings or hemorrhages, it relaxes or stimulates muscles (depending on the
location of touches), improves the nutrition of the tissue and mitigates pain.
Through reflex reaction and through change of the blood supply the massage
affects also other remote organs, and it affects the total organism, which could
have either tranquillizing or stimulating effect. The massage is combined with
the application of local massage agents, most often water and non-irritating
soap, eventually massaging emulsion, oils or suitable crèmes. The partial
massage in the configuration back, arms and lower extremities lasts
approximately 20 minutes, the configuration upper extremities, belly, chest and
neck also approximately 20 minutes, the total massage lasts approximately 40
minutes. The so-called descending total massage is applied in the following
sequence - upper extremities, chest, belly, back and lower extremities, its
effect is rather tranquillizing, the so-called ascending massage is applied in
the opposite direction, and its effect is rather stimulating (increasing the
blood pressure). The classical massage could be optionally combined with bath in
the mirror pool or in the bath. It is subject to the doctor's prescription
(partial or total massage with specification of scale or required locations),
and is also freely saleable as relaxing or stimulating massage. However patients
with diseases
of spine, affecting especially their cervical or lumbar segment should consult
the relevancy of classical massage with their doctor, since in some cases is the
classical massage considered to be impropriate or even counter indicated.
REFLEX MASSAGES
Are a form of the special reflex therapy, where the treatment intervention acts
through nerve connections and endings in the area of selected reflex arcs.. It
is a manual treatment on the body surface, especially in places affected by
disease through reflectory changes. A special technique is gradually applied to
achieve reflectory changes in the skin, subcutaneous area, muscular faciae,
muscles, and eventually in periosteum of some osteal surfaces. The treatment of
nerve endings is done mainly through application of stimulation, mainly by
pulling. Grinding touches with fingertips immersed in the skin or sub cutis are
moved forward, backward, along the imaginary short or long, direct or curved
lines. According to the intensity of the treatment, pressure and slope of
fingers one distinguishes between the following techniques: cutaneous,
subcutaneous or facial. During the connective tissue reflex massage the masseur
directly displaces sub cutis versus base by grasping the skin and sub cutis
between thumbs and forefingers. There are special treatment touches such as
"pushed spiral", two-finger touch, saw-like touch, interspine touch,
pushed vibration, across-the-back spine vibration, interrupted fork-pulling,
etc. Reflex massage is very sensitive body intervention, which besides local
sensations (dull pressure, friction, blushing, etc.) causes also stranded
emotions and reactions (heart beating feeling, short breath, pain in the belly,
pressure in the urine bladder, sweating all over the body, the feeling of sudden
fatigue with an acute need of sleep). This implies the necessity to prescribe
this procedure by a doctor, who should specify the target massage configuration.
There are four basic reflex massage configurations: configuration for the neck
and head, configuration for the back (dorsal), configuration for the chest
(thoracic) and configuration for pelvis. The timing unit is 20 minutes; the
doctor can prescribe also a combination of individual configurations during
individual days. The current combination of reflex massage with the classical
massage is not appropriate, since it leads to the negation of the therapeutic
effect, and one can not exclude the worsening of the patient's condition and a
higher risk of the overall negative response.
TRACTION THERAPY
Is a passive procedure, which
uses mechanical force to induce centrifugal traction in the spine axis, root
joint axis (traction), or in axis of an extremity (extension).
The traction force acts essentially only at the level of soft tissues of the motorics
organs (muscles, ligaments, sinews, joint capsule). Traction pulling can be
induced manually by another person (manual traction), which could be a part of
the whole set of techniques used in the context of individual remedial
gymnastics (mainly manual tractions of the cervical spine, or manual tractions
of the lower extremities in the femoral axis). Similarly during the individual
remedial gymnastics the therapeutist can apply the so-called redressive
positioning, which consists of force (hand, sand bags, belts) applied directly
through the joint, the limited motorics of which we thus wish to intentionally
change. The so-called mechanical traction of cervical spine is done using the
Glisson's cervical traction set with T-bar and applied pulling with a weight, or
on a sloping traction table taking advantage of the pulling force of the
patient's weight caused by gravity. Similarly is applied traction of the lumbar
spine in a position lying on the back, with a side-fixed pelvis (with the
traction option flexion-wise or extension-wise). The pulling force acting during
these tractions is constant and continuous. Tractions in water use again the
pulling of the patient's own body, the influence of gravitation in the water
environment is significantly eliminated by buoyancy caused by hydrostatic
pressure. The so-called instrument-assisted tractions are performed using a
special equipment, which works in the mode of intermittent tractions, with
pre-set not just traction force and its total duration, but also duration of the
dynamic pulling and resting interval. Modern equipment has pre-set special
programs. The procedure lasts 20 minutes and has to be prescribed by a doctor,
who specifies the type and location of the required traction.
YOGA EXERCISES
Is
carried on in group, under the guidance of a trained facilitator. The main focus
is to achieve a better harmonization and synergy
between individual body systems and functions, inducing psychic as well as
physical harmony,
enhancing physical as well as psychological
resistance. The organism is trained in the context of the unity of body, mind
and emotions. Hatayoga has the best elaborated training compositions, which
through static/dynamic activities affect not only
the motoric system, but also internal organs and the central nerve
system. The main effects of the yoga exercise could be summarized into the
following effects: relaxation, gradual adaptation to the reflex stimulation,
changes in intensity and distribution of the stimulation, and inhibition in the
central nerve system, changes in the rhythm, stabilization of the regulatory
processes, training designed to control autonomous
functions, the use of negative induction on the mediated inhibition of some
systems, and therapeutic auto-suggestion focused on narrowing the consciousness.
The main exercise
categories include asana (positions), bandhas
(closures - predefined configurations of muscles), mudras (similar to bandhas
but with symbolic content), purification procedures (kriya), breathing exercise
with holding one's breath (pranayama) and mantrayoga (meditational syllables and
texts). The
time unit applied is 20 minutes.
NORDIC WALKING
Is a dynamic fitness walking in a terrain with different demand ness profile,
where the walker uses a pair of special sticks for Nordic walking. The rhythm of
movements is determined by a cross-walking dynamics. This is an ideal form of
physical exercise with an efficiency higher by 40 - 50% then during the normal
walking. The walker has a better stability and feels more secure, load imposed
upon load-bearing joints is by 30% lower, and the Nordic walking is thus better
tolerated in cases of degenerative diseases of the spine and load-bearing
joints, and it is one of the most suitable motoric activities for patients
affected by osteoporosis. It improves the fat and carbohydrate metabolism, and
thus could be used as an effective outdoors exercise for reducing the body
weight. It is also an efficient breathing gymnastics, improving cardio-pulmonal
efficiency, being a good form of cardio-pulmonal exercise. The Nordic walking is
an ideal motoric activity for all age groups, including seniors with
degenerative diseases of spine, load-bearing joints,
balance-stability-co-ordination impairments, or ischemic vascular diseases. It
should be prescribed by a doctor, procedure is designed for 60 minutes.
Individually adjustable size of Nordic sticks are fit for each patient and
borrowed to him/her. The procedure takes place in the natural environment of the
Spa Island. We recommend sport/outdoor dress and conformable sport shoes.
During the rainy weather the procedure takes place in the gymnasium as a
special exercise NW Stretching.
REHABILITATION SWIMMING
Belongs to most important sports for physically handicapped, which only supports
the idea of its importance in rehabilitation. It also takes advantage of
positive effects of water, both - thermal, as well as mechanical, induced by the
hydrostatic pressure, water resistance and lifting
of the body under the water. The basic styles are breast-stroke (the main
driving force are lower extremities, this style is suitable for better mobility
of lower extremity joints), crawl (the main driving
force are upper extremities), back-stroke (this style is suitable especially for
impaired body position, diseases of spine and lower extremities) and the
butterfly (recommended for not yet fixed kyphosis at initial stages of
Bechterev's disease). The styles breaststroke and butterfly are not suitable for
cervical spine diseases with overloaded spine muscles, and neither for patients
with impairments of intervertebral discs, for the latter the more suitable
styles are backstroke or crawl.
MECHANOTHERAPY
In a narrow sense of the word uses various static as well as dynamic forces,
applied through the use of different equipment, tools and devices designed for
curative/rehabilitation purposes. The objective is mainly achievement of larger
joint mobilization spans using technical means designed to increase the joint mobilization
(e. g. biking on the fitness concept machines or ergo meters, mobilization of
arm joints through exercising on wall bars, exercising with electric dynamic
splints, relaxation of cramps (spasms) using special equipment with antispastic
devices - antispastic kinesiotherapy Rack Motomed). These devices can act either
passively (i. e. controlled with patient's own forces), or actively, using the
equipment drive with an option of varying different parameters set up (speed,
frequency, resistance, direction of the motion). Further purpose of
mechanotherapy is the strengthening of selected muscle groups, which is achieved
mainly through various resistance-weight systems, electromagnetic induced
resistance, or resistance induced with elastic belts or springs. Many devices
have universal character and at the same time improve the mobilization of joints
and muscles. The procedure has to be prescribed by a doctor; time unit applied
is 20 minutes.
FITNESS
CENTRE
Is
a modern unit, which has many in common or similar with mechanotherapy. However
it is focused mainly on the targeted and professionally guided fitness training
of individual muscle groups. Individual fitness machines are always designed for
different muscle groups, which are in most cases involved in a certain isolated
movement (e. g. stretching arms, extending arms, bending down, straightening,
internal and external rotation, etc.). The procedure has to be prescribed by a
doctor (which should specify main objectives and recommended
courses), but it is also freely saleable, applied time unit is 20 minutes.
MANIPULATION
AND MOBILIZATION THERAPY
Are techniques of the manual
medicine. In most cases they are used for the treatment of functional joint
blocks, which are painful and restrict the movement, and cause additional
pathological reflectory changes, mainly aching points, zones of hyperalgic and
muscle spasms. These are in most cases the so-called joint areas of spine, Le.
the joint connecting head with 1st vertebra of the cervical spine, 1st and 2nd
vertebrae of the cervical spine, cervical/thoracic, thorax/trunk and trunk/lower
back. The purpose of the treatment is the release of the motoric segment (in the
are of spine), or the restoration of the joint freedom (in joints), but also the
adjustment of the muscle tone, and treatment of the so-called trigger points,
which many times cause I the pain, muscle weakness or limited motorics.
Manipulation has to be done I exclusively by a doctor, specially trained and
licensed. The idea is first to create the so-called overstress and then to
release the block through the fast movement directed in the designed direction,
small in scale and using a small force. The mobilization results either in the
mitigation of the block prior to the subsequent manipulation, or in a direct
elimination of the block. It applies the principle of short repetitive movements
starting from marginal to neutral position, without impact, but with a permanent
overstress. Effects caused by the method could be magnified. through
the combination of the muscle facilitation and inhibition. However the method
requires the co-operation of the patient, which exerts an isometric pressure in
the opposite direction, and its relief results in relaxation, which allows
partial movement in the direction of the block (Mitchell). The application
should be repeated several times. In order to exert the isometric resistance,
the therapist can also take advantage of gravitation force of head or extremity
(ant gravitation relaxation - Zbojan). Selected simple mobilization techniques
may be also applied by a trained physiotherapist (slower muscle pulling -
release, subsequent muscle extension - stretching, post facilitation relaxation
or inhibition) in combination with the so-called soft techniques. This is a
non-standard, specially covered treatment procedure.
PHOTO THERAPY
a) Ultraviolet rays is
a part of the sunlight,. in photochemical and biological sense very active. The
component UV-C (wavelength < 280 nm) IS absorbed in upper layers of
atmosphere owing to the ozone layer. It has bactericide nature. The sunshine
component UV-B (315 - 280 nm) is the main cause of sun-induced erythema and skin
pigmentation. The component UV-A (400 315 nm) is more energy-poor, and because
of its better penetration it has curative importance. UV rays with the
wavelength 250 - 300 nm is involved in the production of vitamin D. Cautiously
applied UV light can improve the body immune response, muscle performance, and
it supports the creation of red blood cells. On the other hand in a long-run
UV-C light and also some other components have carcinogenic nature. Modern
solariums are the combination of multiple UV and infrared radiation. They are
good for the prevention of rachitis, poor health condition, fatigue, anemia,
some neuralgia and a set of allergic and skin diseases (special lamps).
Application has to be designed by a doctor, it lasts from 30 seconds and after
gradual increase (by 1 minute) could be applied up to 10 minutes (which is
strictly individual).
b) Laser therapy uses coherent, monochromatic light - part of the segment of
visible light or microwaves. The laser beam is the carrier of energy, which it
can deliver after falling on the illuminated plane. The Helium-Neon laser
applies the red light with the wavelength 632.8 nm. The diode-based therapeutic
lasers have the wavelength from 532 nm (green) up to 1060 nm (infrared). In
order to affect the superficial tissues it is better to use lasers with a
shorter wavelength, infrared laser affects deeper layers. The laser can be
applied either continuously or in impulses. It has a significant biostimulating
effect, mainly due to the fact that it stimulates collagen,
supports tissue vascularisation, speeds up the regeneration of blood vessels,
lymphatic routes and epithelia, better use of oxygen and glucose, and in
general facilitates the healing process. At the same time it acts as analgesic,
anti-inflammatory agent (antibacterial and antiviral agents), and causes also
the shrinking of oedemas. However the application of laser has also several counter indications
(strict prohibition of application on eyes, on malign and precancer tissues,
banned application on glands, belly during pregnancy, varicose veins or acute
neuralgic attacks). Application is done locally, based on the doctor's
prescription, either on small areas (algic points, initiating trigger points,
treatment of wounds, etc.), or larger areas (using laser scanner). A doctor
licensed in acupuncture can apply the laser in acupuncture
points (laser acupuncture).
c) Bioptronic lamp is the source of curative biostimulating
light, which uses polarized light generated with the halogenous lamp. The light polarization
is a prerequisite for the stimulation of living tissue following the contact
between the light and the tissue, which supports the development of health cells, which is the essence of the biostimulating effect. Bioptronic lamp
(biolamp)
produces polarized light, which is polychromatic, and completely stripped of
the UV component. The light has the wavelength 430 - 2800 nm, it affects mainly
the surface, but also the depth of tissues (approximately 1 - 2.5 cm). The light
supports control processes on the tissue and cellular level, slowing down
pathological and degenerative processes within cells, accelerating metabolic
processes, activating the mitochondria apparatus, supporting the creation of
prostaglandin and other cellular transmitters, accelerating healing of wounds,
skin drafts and scars. It has applications in dermatology, for the treatment of
scars, diabetic complications, bedsores, reflex changes,
oedemas, biolamp supports regenerative processes during the skin ageing. The
duration of application is 4 - 6 min, skin surface has to be duly cleaned and
degreased.

HEAT
THERAPY
a) Paraffin therapy is
the local application of heat, which takes advantage of the latent heat
transmission during the paraffin hardening process. Paraffin hardens at the
temperature approximately 55°C (52 - 56°C), 1 k~ renders 35 kcal. The paraffin
temperature tolerance could reach u~ t~ 60 C. ,There are several application
methods, method of paraffin veils (application of veils submerged in the liquid
paraffin), method of repeated soaking (short-term soaking of e.g. hand in the
paraffin bath, after lifting the hand up there is a thin layer of paraffin on
the skin which starts hardening, the repeating of this procedure results In
creation of the "paraffin glove" - suitable mainly for the
application, on small joints and unflat surfaces), the method of brush
application In layers (similar principle), film method (application of 5 - 10 mm
thick paraffin film a the stage of starting hardening). The local application of
heat affects the skeletal muscles, causing relaxation, releasing spasms and
contractures, easing the chronic pain, improving the local blood hyperemia,
nutrition and healing of tissues, affecting the softened tissue, causing
resorption of chronic infiltrates and oedemas, it has sedative and
immunobiological features. In case of acute inflammations the heat applied
locally increases the pain, supports the development of oedema, and often
worsens the patient's condition. Special precaution has to be taken in case of
seniors with cardio-vascular disease. Local heat cannot be applied on places
with implanted metallic material. The procedure lasts 20 minutes and it has to
be prescribed by a doctor.
b) Piestany Parafango is the original form of local application of heat
through
application of parafango film, approximately 1 cm thick, applied at the moment
when it begins to harden. According to the original technology the, Piestany
parafango is made as a mixture of dry matter from the Piestany mud and a clean
hard paraffin and paraffin oil. The mixture is melted and heated in a special
mixing unit. It is a more patient-friendly local heat load applied on the body,
and since it is not the plain paraffin, the amount of delivered latent heat
during hardening is lower. Part of the heat is delivered to the body through
conduction from the preheated Piestany mud component.
In addition to that the heat is delivered also through direct heat flow and
convection. The temperature of the film at the moment of application is around
50 - 55°C. Indications, effects and counter-indications of this therapy are
similar as those applicable to paraffin therapy. The procedure last 20 minutes
and it has to be prescribed by a doctor. Since the heat load is patient friendly,
the procedure is freely saleable.
c) Dry hot patch (thermophor, lavathermie - hot pack) is the local
application of dry heat through the application of heat carrier. The old
standard thermophor is filled
with a hot water, modern lavathermie or hot pack contains chemical substance,
which
generates heat if activated and reaches up to 54°C. This heat pack could be
repeatedly used for the local application of cold, if precooled down to
approximately 6°C.
d)
Priessnitz's packs
are cold, sweat-generating wraps for local application, mainly for cases of joint problems combined with oedemas and pain
(inflammatory or traumatic etiology), subcutaneous hem atoms, for the relaxation
of skeletal muscles, but also for e. g. chest during tonsillitis, pharyngitis,
or irritating coughing. A wet cold compress is applied on the treated area (a
fabric soaked with cold water, water is partially removed), it is further
covered with PE foil and further with dry, warm (best if wool) pack. During
first 5 - 10 minutes there is a hypothermic phase with vasoconstriction, another
approximately 20 minutes there is an isothermal phase with a switch from
vasoconstriction to vasodilatation, further approximately 20 minutes there is an
ongoing local vasodilatation and hyperemia - hyper thermal phase. The pack
should be kept applied for approximately 1 hour, and could be applied repeatedly.
e) Cryo therapy is the treatment with
cold, which is administered either as
a total or (more often) as local application. For this reason there are several
types of cooling compressors available on the market. Most often they are cryo-packs,
containing the pearl cellulose, which is cooled down to -18°C and it still
keeps well shapeable consistency. The similar effect could be achieved with also
gel cryo-pack (hydrated silicates), similarly cooled down to -18°C. The local
application of cold will induce firstly vasoconstriction, leading to the
inhibition of muscle spasms and muscle hyper tone, mitigation of pain, improved
mobility, and at the same time to slower progression of nerve impulses,
mitigation of inflammation and stimulation of immune response with
anti-inflammatory effect. There is at the same time a better conduction of heat
from deeper layers as well as convection of heat through the blood vessels. It
is very important that after each cryotherapie administration there is an active
motorics-oriented therapy (active exercise), which strongly supports an active
vasodilatation. In addition to this the exercise is easier for the patient
(inhibited muscle spasms and pain). Indications are wide - fresh-posttraumatic
states, active inflammation of joints, mitigation of spastic conditions in
neurology, painful diseases of spine with paravertebr-al spasms, or individual
treatment with curative gymnastics. The procedure has to be prescribed by a
doctor, which shall determine the location, number of cryo - packs and
application time. The procedure is contraindicated in case of disorders of
arterial blood supply, allergy to cold and other forms of cold intolerance.
INHALATION
Uses inhalation of liquid or solid agents dispersed most
often in the air (the so
called aerodispersoids). These are mainly aerosols (gentle fog up to 10 microns)
or sprays (coarse fog, 10 - 40 microns). Most often used are aerosols inhaling
devices or ultrasonic inhalators. The use of mineral water with the sulphur
content (H2S, HS', S20t , sot) has a local antiseptically action, which is
caused by the sulphur in oxidized of reduced form. Sulphuric waters equally
reduce the volume of secretions during hyper secretion. H2S concentration should
not exceed 0.006%. Very effective is the use of weak sodium chloride waters
(salty waters), which act as expectorant agents, making the phlegm less viscous
and making the coughing easier. lodine/brome containing salty waters contain
ions of iodine, they act as mucolytic agents, being expectorants and anti-inflammatory
agents. Inhalation therapy is suitable especially for the treatment
of chronic inflammatory diseases of upper and lower respiratory infections, and
bronchial
asthma.
SPECIAL
PSYCHOTHERAPY
Is a special care
offered to patients individually by the clinical psychologist. Short-term
interventions are focused mainly on handling the acute crisis situations, when
the bad condition of the mind adversely affects the pattern of practically all
clinical diseases, and thus the course of the spa treatment and its final
effect. Such conditions include depressive states, different form of
neurotisation and patient's psychological misbalance, which could be the result
of situations experienced under acute stress, or chronic diseases. Part of the
curative psychotherapeutic sessions is also autogenously exercise, and
instructing the patient in order to give him/her strength to overcome current
forms of psychological stress, and to generate positive psychological relaxation
in the social environment of his/her own living space. Part of the psychological
care is its diagnostic stage, when the psychologist performs the evaluation of
the psychological condition of a patient or the analysis of his/her personality.
Psychotherapy is offered individually, but also as a group therapy with
relaxation focus. The procedure should be prescribed by a doctor, and delivered
by a clinical psychologist. It should be mentioned that the spa environment
itself is an efficient psychotherapeutic tool, being a mutual symbiosis of
culturally advanced and social environment of the spa city, with a unique
natural ecotype of the Spa Island and its neighborhood. Higher frequency of
social contacts and cultural events is sensitively combined with the treatment program
of spa guests, significantly relieving the physical as well as mental
recreation, and regeneration of strength.
MEDICAMENT
TREATMENT
Is a part of the
spa care. Patients, who use drugs prescribed by their family practitioners for a
long term continue to do the same during spa stay. However in many cases could
be the possible number of drugs used during the spa stay reduced. This includes
mainly the drugs used to reduce pain, muscle tension, or various
psychopharmacological agents, and sometimes also drugs against high blood
pressure, because the relaxing spa regime, life style without agitation, and the
influence of many spa procedures lead to the gradual drop of the high blood
pressure. Similarly, in case of temporary painful reactions, e. g. within the
framework of the so-called thermal crisis, the doctor on duty tries to handle
such difficulties preferentially through the use of physical medicine and reflex
therapy. If the patients' health condition requires so, patients during the spa
stay are provided with the necessary medicament treatment.
|
MUD
BATH
Is a
unique in the world pool bath, with a natural bed made from 10 - 30 cm
layer of original Piestany sulphuric mud, with
penetrating hot thermal mineral
water springs, and the water layer reaching
approximately 50 - 90 cm above the sulphuric mud. The temperature of the mineral
water varies from 39° to 40°C, sometimes reaching locally higher levels in
places above the springs. Patients in the mud pool stand, slowly walk or sit on
the edge of the pool. They can practice the mobility of their joints, mainly of
upper extremities, application of the warm mud on the body, etc. The duration of
the bath is strictly individual, in
average it lasts 5 - 8 - 10 minutes, and is
subject to the doctor's prescription.
The procedure ends with a shower and up to 15-min
dry wrap. The doctor on
duty can prescribe a combination of this procedure
with a bath in the mirror pool
and the follow up classical massage.
MIRROR
POOL
Is a pool with artificial bed, filled with natural sulphuric thermal
mineral water with temperature 38° - 39°C, where the patient is submerged up
to his/her arms, or he/she can sit on the poll edge. The mirror bath, and its
duration is subject to the doctor's prescription, it last 10 - 20 minutes and
ends with the dry total wrap up to 15 minutes. Patients are under the permanent
supervision. If a patient's conditions allows that, the doctor on duty can
prescribe a combination of consecutive baths in the mud pool and in the mirror
pool. Some mirror pools have a temperature of the mineral water that was reduced
down to 37°C. This configuration of procedures is freely purchasable (it is known as folk bath).
DRY CARBON BATH
Is
applied for the whole body or for extremities and in its nature it completely
differs from the water bath. The medium used is 95 - 100% carbon dioxide moisture
with sweat within a closed space in PE bag, with the heat accumulation inside.
Effects: stronger hyperemia of the body organs, brain and muscle tissues as a
result of vessel dilatation, including capillaries, better blood flowage
characteristics, better elasticity of erythrocytes, better generation of an
intracellular glycogen, better coronary blood supply into the myocardium, better
efficiency of the heart action, drop of the blood pressure, better blood supply
into extremities, less new swellings, positive impact on varicose complications,
positive effect on the cellulite skin, hyper pigmentation, emerging old-age coetaneous
nevuses, better recovery of skin defects and decubitis ulcers, ulcus cruris,
burns, scar softening, faster recovery of skin. The patient's body is enclosed
in a special plastic bag, with inflated approximately 125 I of carbon dioxide,
of which some 2 I will be taken in through skin surface resorption, which
significantly improves when the skin is hyperemic
and then moistured. The application time is 40 minutes. The procedure requires
rest, eventually
accompanying relaxation music. Operator must make
sure that the patient does not inspire gas (during filling up the "bath
itself", and subsequent release of
gas). The procedure is suitable for all indicated
groups, especially for patients with diseases of heart and blood vessels,
including vasculatory diseases of brain, hypertension, varicose vein
inflammations, vasculatory complications of diabetes, osteoporosis, spine
diseases, but also post surgery, posttraumatic or rheumatic diseases of
locomotive organs. This is a non-standard, special
treatment procedure, which is included in some of
the several special treatment programmes.
WATER
TREATMENTS
a)
Sub-aqua manual massage (underwater
massage) is applied to a patient, which lies in
a 37°C (36° - 38°)
bath in a multifunctional tub, or in a Hubbard's tank. The massage itself is
performed using the energy of the water stream, which outflows with the pressure
0.2 - 0.4 MPa (Le. 2 - 4 atm). The water stream is applied on the body manually,
from the distance 10 - 15 cm and under the angle 30° - 60°. By varying the
distance and the application angle it is possible to change the intensity of
massage. The application on extremities should be done from the periphery
towards the body centre; the reverse direction should by-pass the body. On the
belly the direction should follow the course of the large intestine, oh the
chest and back the applied pattern should have the shape of repeated horizontal
eight. The pressurized water shall not be applied on genitals, female breasts,
and the area of the heart. The duration of the procedure is 15 minutes. The
massage acts mainly to release the stiffness of muscles and connective tissues,
to mitigate the pain and to partially release the joints. The procedure is not
appropriate for bleeding conditions, skin and subcutaneous tumors, pregnancy,
renal diseases, varicose veins, vein post-inflammation conditions, hernia in the
region of belly, post-operational scars, conditions following the heart failure.
Underwater massage could by provided in a hydro massage tub, where it is applied
using larger number of jets of different size and diameter, which operate either
stationary or they are controlled with special software. Manual underwater
massage has to be prescribed by a doctor; hydro massage bath (whirlpool) is freely
saleable.
b) Scottish sprays are a very
radical, strongly stimulating and irritating procedure, based on the contrast
application of cold and warmth. It is applied using the water flow from jet with
pressure 3 atm, from the 3 - 4 m distance. The spray starts with 38° - 42°C water applied during 30
seconds, followed by the spray with 16° 18°C water applied for 5 - 10
seconds. The spray is repeated several times, first in the area of upper and
lower extremities, then on the torso and belly (following the course of the
large intestine), always from the frontal as well as rear side. The procedure is
not applied on the area of breasts and genitals. It ends with the cold spray.
The whole application results in a significant hyperemia of the skin and feeling
of the well-congested and warm skin. The procedure increases the immunity,
supports the metabolism, facilitates the restoration of vegetative and
functional malfunctions, and stimulates immune response. The procedure last 15
minutes, and it has to be prescribed by a doctor.
c) Whirlpool bath
is a
gentle massage, using the whirling water, either through its partial application
on upper of lower extremities (whirlpool bath of upper extremities, lower
extremities), or through the total body application (total body whirlpool bath)
in a tub with 1/2 or 3/4 water fill with isothermal temperature (36°37°C).
The water in the bath is whirled using the electrically operated water turbine.
It contributes to a better blood flow in extremities, activation of skin
receptors; and release of muscles and joint stiffness. The
procedure lasts 15 minutes. The total body whirlpool bath can be substituted
with a similar hydro massage tub or Jacuzzi pool. The procedure has to be
prescribed by a doctor, and is also freely saleable.
d) Stepping bath - "alternating leg
bath" is an active movement/ hovering in small tubs, filled
with water with contrast temperatures. It starts in the hot water (40° - 46° -
50°C) lasting 1 - 2 minutes, then the patient steps in the cold water (10° -
16°C) for 15 - 30 seconds, and the whole cycle is repeated 6 - 10x. The
procedure always ends in the cold water with subsequent drying with a bath
towel. The procedure has positive impact on headaches, migraine, vegetative
disorders, disorders of the blood circulation (cold feet), it is an excellent
vein gymnastics for varicose at initial stage, supports the loosening of ankles,
leg joints, and it is suitable for use after traumas of these body parts (feet).
The procedure lasts 15 minutes, it has to be prescribed by a doctor, and is also
freely saleable.
FUNCTIONAL
REHABILITATION DIAGNOSTICS
Is designed to make special evaluations of the functional capacities of the motorics
system, e. g. muscle test and goniometry of extremities, walking test,
diagnostics of neurological, motorics and communication capacities, capacity
testing of spine, test of the hand, capacity index of patient's
self-sufficiency, some further specialized tests (Parkinson's disease, paralysis
of facial nerve, etc.). This diagnostics registers the status of tested
capacities in the beginning of patient's stay and their subsequent change
throughout the course of balneorehabilitational course. It is subject to the
doctor's prescription, and is used mainly in case of more complicated diseases
of the motorics organs, performing mainly the purpose of impartial examination
of a changed health status of patients. The diagnostics should be carried out by
a professional physiotherapeutist (rehabilitation worker) as the follow up of
the procedure called individual therapeutic/remedial gymnastics.
INDIVIDUAL CURATIVE PHYSICAL EDUCATION
Represents a special kinesiotherapy, which uses motion for the recovery or
reparation of kinetic capacities of the rehabilitant. It is carried out either
individually, or in groups, and there are many different types of it.
a) Individual curative
physical education is
performed by a physiotherapeutist, who individually treats the patient. It is
designed to maintain or restore physical motorics capacities, as well as to
develop functional motorics adaptation of the organism. Prevailing are special
exercises of individual disabled parts, training of basic motorics stereotypes,
provided they had been disabled by a disease or injury, damaging the capacities
of bones, muscles, joints and nerve system. Especially for cases with weakened
muscles, stiffened joints or other defects is combined with passive exercising,
positioning, spling traction, or other special methods. Inseparable part is the
rehabilitant's participation in an active exercise is his/her proactive
involvement in it, and co-operation during exercise. Used methods include also visualization
exercise, active muscle contraction, active assisted movements, free active
movement, or active isometric exercise. An important part of the procedure is
the fitness exercise, strengthening, correct breathing training, correct body
position training, correct sitting position training, correct standing position,
walking and correct motorics stereotypes during daily activities. Individual
curative gymnastics uses the whole set of specific methods, targeted based on
the indicated therapy (e.g. Kenny's analytical gymnastics, Kabat's nerve-muscle
facilitation, training of Mensendieckova's correct motorics co-ordination,
Klapp's or Fay's crawling, or reflex crawling and Vojt's locomotion or Bobaths'
facilitation of motorics patterns). Procedure duration is 20 minutes; procedure
is subject to the doctor's prescription.
b) Sling exercise therapy
(the
so-called self locking function rehabilitation concept) is a special format of
individual remedial gymnastics, which excludes the influence of gravity, i.e. it
is an exercise with the elimination of the weight of exercised segment of
extremity. The method is suitable for cases of weak muscle strength, if the
person cannot overcome the resistance of gravity. It allows the use of
relaxation elements, patient-friendly mobilizing techniques, patient-friendly
dynamic traction, training of the muscle stability, sensomotoric exercise,
cautious increase of motion span, exercise within open and closed kinetic cycle.
The method improves the maintenance of stability and morphological integrity of
segments, global regulation of the body position, better capacity of fast
physical response and capacity to impose strength. During this exercise we use a
high-end state-of-the-art rehabilitation equipment Nordisk Therapy Master
device. The procedure lasts 20 minutes and has to be prescribed by a doctor.
SOFT TECHNIQUES
Are a
special method of manual medicine, which affects reflex changes in muscles and sub cutis,
designed to reduce pain and muscle tension, and facilitate the subsequent mobilization
or manipulation. Soft techniques should be applied by a physiotherapeutist, and
this method applies several techniques, which could be partially compared to
massage elements, mainly to those used for the reflex massage. They include e.
g. the skin fold, which is very painful in places of reflectory changes, and the
method is trying to gradually reduce its resistance, or connective tissue
massage applied with a soft pressure and finger tips movement with fingers
immersed in the skin. Another method is based on skin elongation and the gradual
elongation of skin through making folds in places with hyperalgic alterations.
Also used is the soft touching the skin with a finger. Soft techniques could be
prescribed as an independent procedure (time
unit 20 minutes), often being applied in the context of individual remedial
gymnastics.
MANUAL LYMPHATIC DRAINAGE
Is a highly specific
method, during which the therapist, using a series of gentle
stroking pressure and other manual techniques
facilitates the faster return of venous liquids into the treated extremity. The
procedure results in the evacuation of capillary vessels followed by an influx
of arterial blood (suction principle), with the most important impact - a better
lymphatic draining. Better circulation and improved evacuation of lymph from the
extremity results in smaller oedema and the subsequent reduction of chronic and
lymphatic swellings, as well as better blood supply and nutrition. The method is
used mainly for the treatment and prevention of chronic Iymphoedemas of upper
and lower extremities (e.g. after the mastectomy). Inseparable part of this
procedure is the follow up bandage of the extremity and its positioning. The
procedure lasts approximately 30 min / 1 lower extremity, and it shall be
prescribed by the doctor. It is classified as a nonstandard, specially covered
treatment intervention. The lymphatic drainage can be performed also using such
equipment as pneumatic vacuum massage device (using the suction principle
induced through uninterrupted negative pressure), or overpressure massage device
(using the principle of interrupted overpressure in an inflatable pneumatic
splint), or a combined vacuum/overpressure massage device (using the principle
of alternating negative pressure and overpressure in a
special cylinder inside the device).
GROUP KINESIOTHERAPY
Which is an expert-guided active curative gymnastics in a gym, with a group
of rehabilitants with a similar diagnostic profile. Examples: group kinesiotherapy
for diseases of SPINE, LOAD-BEARING JOINTS (hips or knees), group kinesiotherapy
designed for OBESITY, OSTEOPOROSIS, for RHEUMATIC DISEASES, for VASCULAR
DISEASES OF BRAIN, for CARDIAC etc. The exercise uses elements designed to relax
overloaded muscles, strengthening of weak muscle groups, mobilization of joints
and pelvis, strengthening of static/dynamic functions necessary for the correct
body position, and training of correct motorics stereotypes. Inseparable part of
the exercise is the breathing gymnastics, as well as training of speed, agility,
endurance, balance, strength and overall fitness improvement. The time unit
applied is 20 minutes.
REHABILITATION PATH
- SCHOOL OF
WALKING
Is a
special method focused on the restoration and training of the basic human
motoric stereotype - walking with erected position
of the body. Rehabilitation is designed based on the walking impairment (costal
component, connective tissue component, nerve/muscle component, and combined
forms). This determines the type of the walking exercise without any load, with
partial load or full load. The patient is learning to use different supportive
aids (verticalising frame; path with safety rail; underarm, French, Canadian
crutches; multipoint bars; "baby" walkers, or different other, more
complex prosthetic aids). Part of the walking training is focused on overcoming
architectural barriers, walking in difficult terrain, on a sloped terrain, etc.
The procedure is performed under the control of the specially trained
physiotherapeutist, it has to be prescribed by
a doctor, and the time unit applied is 20 minutes.
It could be carried out also with a group of patients, under the supervision of
a skilled trainer, guiding each member of the group individually. Walking school
starts in the premises of spa houses and in their rehabilitation facilities, and
then moves to use rehabilitation
sidewalks (sometimes called as healthy sidewalks)
outdoors, in the beautiful environment of the Spa Island.
HYDROKINESIOTHERAPY
IN REHABILITATION POOL
Is an active
physical exercise performed in the water under the guidance of a specialist. The
exercise takes place usually in groups with a similar diagnostic focus. The
procedure takes advantage of the positive effect of the water environment (water
temperature, hydrostatic pressure, buoyancy, and water resistance). The water
buoyancy lifts the patients, and it is used especially for patients with so far
unhealed wounds from accidents or surgery of upper extremities, which are not
yet allowed to fully load the extremity. Movements in the water environment are
often less painful than on the land, the muscle relaxation is more efficient.
The water pressure acts as elastic bandage; water resistance strengthens the
muscle corset. The water environment is very suitable for treatment of static
impairments and incorrect body position, very efficient is the hydrokinesiotherapy,
used for exercises for patients with degenerative diseases of weight-carrying
joints and spine. In water it is possible to train walking, the water resistance
could be increases using the artificial stream. The water temperature in the
rehabilitation pool is approximately 33°C; the pool is 120 cm deep. The most
frequent indication groups for hydrokinesiotherapy are: spine disease group,
group of patients with diseases affecting lower extremities, group of patients
with neurological diseases, geriatric group of rheumatics, etc. The procedure
lasts 20 minutes
and has to be prescribed by a doctor, but it is also freely saleable.
ERGO THERAPY
Is a therapy using the labour activity in order to achieve better
re-education of the body functions. It is used for the objectivisation of the
functional capacity of individual patient (testing of usual activities), which
is necessary for the correct therapeutic specification necessary for achieving
the social adaptation and integration of a handicapped individual, both at work
and in personal life (self-reliance and self-service). Ergo therapy is very good
for strengthening the motivating of a handicapped person. According to the type
of disorder the exercise is focused mainly on exercising the hand and arm, leg,
ankle and curs, but also on exercising the head, neck, chest and spine. Many
times are disorders combined, i. e. disorders of motorics go hand in hand also
with serious disorders of mind, cognitive functions and speech. In indicated
cases this is an additional reason for necessary co-operation between ergo therapy,
psychologist and the speech therapist. Ergo therapy shall be carried out by a
specially trained the so-called ergo therapist, it should be prescribed by a
doctor, applied time unit is 20 minutes.
ANTISPASTIC
KINESIOTHERAPY
Of lower and upper
extremities is carried out using a special, electronically controlled device
RECK MOTOMED (MOTOmed VIVA), with original feature anti-spasm functionality,
taking advantage of the principle of alternating phase of the muscle-antagonists
movements. Antispastic kinesiotherapy always switches to the follow up training
of walking. The device applies dynamic kinesiotherapy to lower and upper
extremities, in case of minimal muscle strength or large "elasticity",
which the is patient unable to overcome himself/herself, it uses electric motor,
and the device acts partially also as a dynamic splint. The advantage of the
device is its ability to record any active movement which the patient performs
using its own strength. The patient should strive and be motivated to exercise
active movements, and the device facilitates such efforts. The biggest advantage
of the device is its anti-spasm action. If confronted with spasm the device
stops moving and automatically moves back. This results in "the
dissolution" of a spasm and gradually in a reduced frequency of spastic
complications. It allows a more extensive dynamic antispastic kinesiotherapy,
better self-reliance of the patient, and most of all the follow up
verticalisation and training of walk. This anti spastic effect supports ergo therapeutic
program, walking school and other activities. Antispastic kinesiotherapy at the
same time supports trophies and mobilization of basic joints in the lower or
upper extremity. It supports also the strengthening of muscles. The device is
designed solely for neurological
indications, and it does not serve as a replacement of ergo meter fitness
concept machines, or ordinary bikes.
CARBON INSUFFLATIONS
Are
subcutaneous injections of medical synthetic carbon dioxide (CO2 ). In order to
apply gas from the pressure flasks method uses a special device with reduction
valve with pre-set outlet pressure 75 kPa, special filter and own applicator
which acts as an automatic syringe with electronic regulation and regulation of
the gas volume. Needles used are disposable needles. Max. application pressure
is 0.7 kp/cm3, i.e. approximately 69 kPa. Applied volume per single injection is
20 - 100 ml of gas, in average 30 - 40 ml per single injection, recommended
number of injections is 6, total number of applied gas up to 200 ml. The
procedure is subject to the doctor's prescription, and the latter shall specify
application locations, application doses of gas, and number of injections. From
the sub cutis CO2 resorbs very well into the venous and lymphatic circuit,
causing artificial local oxygen debt with very efficient and active local and
overall over flush, significantly improved oxidation, and tissue nutrition
supports the creation of blood collaterals, improves the elasticity of collagen fibers,
reduces pain, and makes exercise easier.
GAS INJECTIONS - PNEUMOPUNCTURE
Is an administration of a minimum doze (1 - 5 ml) of medical carbon dioxide
into acupuncture points (also called pneumopuncture). It is one of the ways of
influencing the organisms through stimulation (mainly mechanical, or using other
types of stimulation) applied in specific points on the skin surface. It is
applied especially to pain syndromes of any etiology, and if necessary they
affect the muscle tone, muscle spasms or initiating points. The method is often
used for neuralgic indications, par esthetic conditions and for several other
indications. The local administration of carbon dioxide amplifies the affect of
acupuncture, making the stimulation of acupuncture points stronger and longer.
The stimulation of acupuncture points could be performed also using lasers, the
so-called laser acupuncture. Only a practitioner licensed to perform acupuncture
can apply the method. It is a non-standard, specially covered treatment
intervention.
ACUPUNCTURE
Classical acupuncture,
several thousand years old, is one of the oldest and
most favorite methods of the traditional Chinese medicine. The oldest textbook
described 365 treatment points on the body surface, in 1026 there were already
some 600 described acupuncture points. Acupuncture is its essence inhibits the
pathological dominant feature in the body, brings tone and vegetative body
system response into the normal condition, and brings to norm different
vegetative disorders. It is indicated especially in case of various neuralgia,
headaches, migraines, aching conditions of spine and joints, and in series of
vegetative disorders. Only a practitioner licensed to perform acupuncture can
apply the method. This is a non-standard, specially covered treatment
intervention. Stimulating the acupuncture points in case of classical
acupuncture is performed with special acupuncture needles. Another possibility,
used mainly in countries of Eastern Asia, is application of moxa heat. In our
environment often applied method is the stimulation with a special palpation
technique (acupressure), which the patient can apply himself/herself (self acupressure).
Acupuncture points could be stimulated also using percutaneous stimulation with
electric current (electro acupuncture), or by transcutaneous
electro-neurostimulation applied through introduced acupuncture needles
(AKU-TENS). Stimulation of acupuncture points could be done also using laser
(laser acupuncture), or using a small dose of the medical carbon dioxide (pneumonic
acupuncture).
ELECTROTHERAPY
Uses the curative affect of different forms of electric
energy, it can be also used for electro diagnostic purposes. The administration
is done using different modern electronic devices. Electrotherapy to large
extent uses its analgesic effects, often improves the blood circulation,
nutrition of tissues, mitigates inflammatory activities, and thus facilitates a
better recovery of motoric functions. The duration of majority of
electrotherapeutic procedures is 15 minutes.
a) Galvanic therapy (galvanization, ionophoresis,. four-chamber bath) uses the
flow of the galvanic (direct) current through the tissue, which causes an
active hyperemia, improved metabolism in tissues, shrinking of oedemas and
exudates, mitigation of pain. The procedure is indicated for all aching
conditions of the motoric organs, of any - posttraumatic, inflammatory or
functional origin, except the presence of metallic materials in the current
route and cases of sensitivity disorders. It is applied as a single in-depth galvanization
(using the protective solution for cathode and anode), or as a ionophresis (one
of the clearly defined electrodes introduces in the surface layers some remedial
agent, prepared in the format of an ionic solution, e. g. ionophoresis with
procaine, calcium, hyaluronidase, iodine, salicylate), or as galvanic bath of
extremities (known as hydro galvanic or two/four-chamber bath). The application
of the galvanic current is done in the water bath, with extremities submerged,
each in a separate tub, connected to cathode and anode. The negative electrode
causes rather the increase of nerve agitation, the positive act more as a tranquillizer.
The connection of upper extremities to the anode and lower one to the cathode
acts as descending, tranquillizing galvanization, connection vice versa is
typical for ascending galvanization with an effect of increasing the muscle
tone.
b) Ultrasound uses electric energy of high-frequency currents, which are
converted to the mechanical and heat energy. Ultrasound is a longitudinal
mechanical oscillation with a frequency 0.75 - 3 MHz (most often 0.8 - 1.2 MHz),
and with intensity maximum up to 3 W/cm2. Some 30% of this mechanical energy,
which causes sort of micro massage in tissues is transformed into heat.
Ultrasound supports the excitation of molecules and subsequent chemical
reactions, it improves the blood circulation and nutrition of tissues, tissue
diffusion, acts as spasmolytic agent, with resorbing, analgesic and
antispasmodic effect. The depth of the impact varies between 2 - 6 cm. However
ultrasound causes also remote reflex effects. The basic administration form is continuous
or pulse application, the contact between the head of the device and
tissue has to be supported with the contact medium (paraffin oil, or conductive
gel), there are also heads for sub aqueous application. The place of application
has to be prescribed by a doctor, generally it is either the place of ache, or a
respective segment, with the respective spinal cord root, or nerve route. a

c) Interferential streams act based on the principle of using
interference of the two medium-frequency streams within the tissue, where one
circuit has a constant frequency and the other has adjustable, fluctuating one.
The advantage of such approach is the smoother overcoming of the skin tissue
resistance, good tolerance towards small sensorial and motoric stimulation, and
inducing the effect of interference within the tissue layer. It affects directly
muscles, nerves, improves blood supply (hyperemia) and nutrition of the tissue,
all of which is caused by vasodilatation, analgesia and restoration of the
tissue tone. Depending on the pre-set frequency and its modulation one can
achieve either effects of motoric stimulation/irritation (up to bringing the
muscles into the so-called muscle gymnastics condition), or, on the contrary,
achieve the decline of an increased muscle tone, or cause the more significant
analgesic and spasmolytic effects, or oscillate between stimulating and
mitigating effects, causing the resorption of oedemas and exudates. The
procedure has to be prescribed by a doctor, which should specify the location
and necessary application parameters.
d) Diadynamic streams are mixed
streams, which use effects of the concurrent application of galvanic and
faraday, or other impulse-like streams. This results in combined effects of both
types of streams, especially induction of hyperemia and analgesia. The galvanic
stream component is combined with impulse component. Sinusoid-like impulse
streams have inhibitory or facilitation effect, which depends on the frequency,
intensity and other parameters. The stream with predominantly inhibitory effect
has the frequency 100 Hz, stream with frequency 50 Hz acts rather
dynamogenically, i. e. causing excitation, facilitation and supporting muscle
tone, it eliminates oedema around the nerve envelope, reducing pain and
supporting muscle tone. The procedure has to be prescribed by a doctor, which
should specify the application location and necessary characteristics of applied
type of stream.
e) TENS - transcutaneous electric
electro-neurostimulation - uses the principle that the perception
of pain and conducted pain impulses could be mitigated through stimulation of
nerves on different levels of the nerve system. Used impulses differ in their
shape (symmetrically or asymmetrically biphasic, biphasic- alternating),
they are very short (0.01 - 0.75 ms). They are applied as TENS continual, or
TENS-burst in bursts, or TENS-surge in waves. They are indicated mostly for
conditions requiring reduction of pain (spine diseases, headaches, posttraumatic
pain, and many others). The procedure has ,to be prescribed by a doctor.
f) Impulse therapy
takes
advantage of the curative effect of precisely shaped
current impulses (e. g. sinusoid-like, rectangular, triangular, symmetric,
asymmetric - monophasic, biphasic, etc.). The essential breakdown distinguishes
between currents with rectangular or steep start, and currents with sloped
gradual start. The use is very diverse: rectangular currents are used for the
stimulation of health muscles and nerves (electro gymnastics), for the
prevention of muscle atrophy, strengthening of belly and back muscles,
strengthening of the muscle corset - rectangular angles; sloped currents with
gradual start are used
for the selective stimulation of nerves in cases with peripheral parenthesis,
stimulation of smooth muscles, affecting painful neuralgia, as well as for the
diagnostics of peripheral parestheses using lit curve, rhombuses, chronaxias and
accommodation coefficient. The stimulation also supports hyperemia, tissue
nutrition, if applied as reflex agent may cause relaxation and detoning, having
also analgesic effect. The procedure has to be prescribed by a doctor.
g) Magnet therapy uses the curative effect of the magnetic field on the
biological tissues. The weakest effect are caused magnetic fields which are
static, more intense effects are induced by alternating magnetic fields, and the
most dramatic ones are caused by impulse magnetic fields. In practice doctors
apply different electromagnetic devices, which produce mainly impulse magnetic
fields of certain shape, frequency and intensity. The main effects of the
magnetic field upon the living tissues are vasodilatation, analgesia,
anti-inflammatory effect, myorelaxation and spasmolytic effect, and support of
healing processes and
antioedemic action. Applicators have the shape of hollow solenoid cylinders or
rings, flat applicators, or two-component racked applicators (magnetic
induction, the so-called double decks, used for transregional application).
Magnet therapy has also its counter-indications. These include mainly implanted cardio stimulators,
hemorrhagic conditions, myasthenia gravis, all mycotic (fungal) diseases, which
could become generalized, tumors, juvenile diabetes, pregnancy, active TBC. The
procedure has to be prescribed by a doctor, time unit applied is 30 minutes.
OXYGEN
THERAPY
Is an inhalation of air enriched with 40 - 60% oxygen,
reasonable humid. Inhalation of the mixture is applied using original masks, or
intranasal catheter. Pure medical oxygen is introduced into the body using a
device developed by
prof. von Ardenne (Oxymate). Long-term supply of oxygen eliminates impacts of
chronic hypoxia in the body, improves resistance to the physical load, physical
condition and efficiency of the mind, increased resistance to stresses, and
eliminates their consequences, improves myocardial blood supply, prevents
various coronary and brain episodes, supports brain metabolism, stabilizes the
blood circulation and some other pulmonal and cardiac functions, enhances the
body immune response. The final result of a better resistance to load connected
with the balneo-rehabilitation therapy. The procedure lasts 1 - 2 hours
requires quiet room, comfort and patient's relaxation. During the procedure the
patient receives a ion-vitamin drink. This procedure is a extra one, specially
covered medical intervention.
SPECIAL
LOGOPAEDIA
Is a highly
professional care, offered to patients with different forms of communication
handicap, mainly verbal communication disorders, eventually other combined
forms. The main objective of this treatment is the restoration or reparation of
speech functions. Such disorders came about during the patient's childhood,
either as a result of developmental defects, or in adult age as a result of
sudden diseases (vascular brain episodes and a set of further neurological
diseases and their consequences). Logopaedia care represents a long-term
curative process, which cannot be finished during a single stay in spa. In
indicated cases it requires the follow-up therapy in the patient's place of residence.
The procedure should be rendered by a clinical speech therapist, and prescribed
by a doctor treating the patient.
